Mesh the whole domain; take into account the bunch offset; ippl to 3D mapping for multicore simulations are handled

cleaning up unit tests; make sure to install gtest somewhere and to export the corresponding variable first, there is an install script under tests/tools

- ${CMAKE_SOURCE_DIR}/ippl/src added to include_directories () where required

- define IPPL_SOURCE_DIR

- changes for integrated IPPL library

- add_definitions() replaced with set (CMAKE_CXX_FLAGS...), add_definitions

in sdds file: make sure that the data fits to the type given in the header; here the parameter 'processors' is declared long, but string was given

removing 'using namespace', 'using somenamespace::function' and 'using somenamespace::class' from headerfiles

remove std=c++0x in CMakeLists in test directory; suppress ftree-vrp in build of p3m3d.cpp and PIC3D.cpp
