Improve documentation for ascii2h5block tool
The information about how to run ascii2h5block tool should be improved. The header of ascii input file has been adapted to contain only one line with the grid specifications (OPAL/src#763 (closed)). Additionally, the format of the input field maps is not well described and there is no mention of the compilation flag ENABLE_BANDRF.
