Code indexing in gitaly is broken and leads to code not being visible to the user. We work on the issue with highest priority.

AMR: fix initialisation for sampler and optimiser 

In case of the sampler/optimiser AMR should not be initialised
by 'main' but inside opal.cpp. However, we need to fix the
AMReX_ParallelDescriptor::StartParallel() function, otherwise
AMR hangs.
